Continence Assessments

Our expert team of healthcare professionals is here to provide personalised assessments and tailored treatment plans to help you regain control and confidence in your everyday life.

What to Expect...

NDIS CLIENTS:

  1. Referral Process:

    • You will be required to complete our referral form, which can be filled out by yourself or your support coordinator.

  2. Appointment Scheduling:

    • The team at BrightLives will reach out to you to schedule an appointment via Telehealth (Zoom).

    • A service agreement will be sent to you for signing.

  3. Pre-Appointment Preparation:

    • You will receive an email prior to your appointment containing a list of documents that will assist with your assessment.

    • It is recommended to send these documents before your appointment, if possible.

  4. Assessment Completion:

    • During the assessment, your nurse will gather the necessary information and conduct the evaluation.

  5. Report and Recommendations:

    • After completion of the assessment, your nurse will compile a report along with clinical recommendations.

    • This report will be emailed to you and your support coordinator for review.

  6. Optional Follow-Up Appointment:

    • An optional appointment will be offered to discuss the report and finalize any necessary changes or adjustments.

NON-NDIS CLIENTS:

The process for non-NDIS clients is streamlined compared to NDIS clients:

  1. Appointment Booking:

    • You can directly self-book an appointment with our continence nurse for a consultation and recommendations by following the provided link.

  2. Consultation:

    • During the consultation, you and the nurse will discuss your concerns and needs.

    • The nurse will provide recommendations based on the consultation.

  3. Ongoing Consultations:

    • Following the initial consultation, you and the nurse can discuss if further ongoing consultations are necessary.
